Finding the right doctors, providers, or facilities can be overwhelming. We provide trusted recommendations and work proactively to plan for safety, caregiver needs, and changes in condition to help clients live healthy, fulfilling lives.
Medication Management
Medications can be confusing, from timing to side effects. We help clients safely plan and track their medications, using tools like a customized Medication Administration Record (MAR) and other technologies to make sure everything is taken correctly.
Transitional & Family Support
Moving between care settings, starting therapies, or adjusting to a new diagnosis can be stressful. We help clients and families manage these transitions safely—including arranging services, medical equipment, home modifications, keeping families informed and involved.
Medical Appointments & Advocacy
Scheduling and keeping track of multiple doctors and appointments can be overwhelming. We coordinate visits, accompany clients when needed, and make sure questions are answered. We also help navigate the healthcare system, including insurance or Medicare, so care gets delivered properly.
Crisis Management

Beyond the monthly retainer, you are only billed for the time actually spent working on your behalf — never in preset increments — so you can feel confident that your investment directly reflects the care and guidance provided.
Ongoing Care Management – $180/hour
Personalized oversight, advocacy, and coordination. Time is billed by the minute for fairness and flexibility.
Travel Time – $90/hour
Applied when in-person visits or accompaniment to appointments are required.
Monthly Retainer – 1 Hour ($180)
To ensure continuity and peace of mind, there is a monthly retainer of one hour at $180. This keeps you on the nurse’s radar, with a check-in via text or email each month, even if no active care is needed that month. If care is provided during the month, the retainer is applied toward the first hour of service.
